Forms on Collar are designed to allow businesses to collect necessary information upon booking a service. They are a great way to collect the necessary details you need when customers are registering with your business, booking a service or simply to update their existing information.

Collar automatically fills the client’s and the pet’s profiles with information depending on the answers submitted so you don’t need to copy and past the information. This can include details like address, vet details, vaccination details and so much more.

There are two types of forms on Collar

• Detached: A form that is not connected to any service and can be sent individually to client through a unique link to fill
• Attached: a form that is attached to a service or multiple services and which can be asked at different instances.

Creating a form on Collar

To create a form on collar, follow these steps:

1. Go to the Operations Tab
2. select Forms
3. Select the (+) at the top right corner or the screen to add a new form or select the form you wish to edit from the list.

Options available when creating a form:

• Form Title: The title of the form, this needs to be unique.
• Form type: attached or detached.
• When would this form asked: Select when this form is asked –
• Services: Services to which forms would be attached to.
• Questions: Question which can be included in the form, grouped by category

Deciding when to ask the questions

Collar enables you to choose between 3 different scenarios of when to automatically ask question to users:

• First time any service/product is selected: Collar will show this form to users the first time they book any of your service or purchase any of your products. They won’t see it again.
• The first time selected service or products are purchased: Collar will ask the questions only the first time any of the selected products or services or purchased or booked.
• Every time selected service or products are purchased: Collar will ask the questions every time time any of the selected products or services or purchased or booked.

There is 14 categories of 100+ questions businesses can choose from covering all areas of the pet service spectrum.

To add a question:

1. Select a category
2. Check the question that you wish to ask.
3. Click save

Questions on Collar are designed to ensure the best user experience as well as proper information flow – adding the right information in its right place – like creating vaccination records or updating pet’s profile. Therefore a question cannot be amended. If you have a question that doesn’t exist, please get in touch with us and we will add it.
